A vCard 3.0 is built: formatted name, optional phone, email, and website. When someone scans it, their phone offers to save you as a contact rather than only opening a link.
No. First and last name are enough. Leave phone, email, or website blank and those lines are left out of the vCard.
Business cards, bookmark backs, the last page of a paperback, a signing table tent, or a poster. Keep high contrast (dark code on a light background) and a quiet margin around the square.

The website is baked into the PNG. If the URL changes, generate a new code. For a link you expect to update often, point the QR at a short URL you control.
At least 2.5 cm (about 1 in) square for a simple vCard, larger if you add many fields. Test with your phone before sending a cover or card to print.
